Zephyr will only use the information to provide the subscribed services.

For Care

The information is gathered and stored to provide you access to the sleep studies that you have elected to conduct.  It may be disclosed to other care providers that you’ve indicated to participate in the care and service of your patients.

For Orders and Payment

We may use your information so that the services or products you receive from us may be billed to and payment may be collected from you, an insurance company or a third party.

For Account Maintenance and Engagement

We use your information to create and maintain your account with us.  This may include tracking the use of our products and providing you with customer loyalty or rewards associated with the use of our products.  We may reach out for account engagement through different forms of communication, including, email, telephone, online chat, or other.

For Marketing and Advertising

Your information may be used in order to market our services, business and products to you.

For Customer Service

Your information is used to provide you with customer service for our products or for responses to your feedback, complaints and questions.  We may respond through different forms of communication, including, email, telephone, online chat, or other.

As Required By Law

We will disclose your protected health information when required to do so by federal, provincial or local law.

Any additional use of the collected data, for the purposes of research and development, will be performed on deidentified data.

Where a Reseller agreement or other business relationship document exists, such a document has precedence over this Privacy Policy.

Other uses and disclosures of private information not covered by this Notice or the laws that apply to us will be made only with your written authorization. If you provide us written authorization to use or disclose your information, you may revoke that authorization, in writing, at any time. Right to Inspect and Copy

You have the right to inspect and copy your information. Usually, this includes medical and billing records. To inspect and copy this information, you must submit your request in writing to the Privacy Officer. If you request a copy of the information, we may charge a fee for the costs of copying, mailing or other supplies associated with your request. Records of protected health information will not be maintained for longer than is necessary to provide the subscribed services.

We may deny your request to inspect and copy in certain very limited circumstances. If you are denied access to protected health information, you may request that the denial be reviewed. We will choose another licensed health care professional who was not directly involved in the denial to conduct the review. We will comply with the outcome of the review.

Right to Amend

If you feel that the information we have about you is incorrect or incomplete, you may ask us to amend the information. You have the right to request an amendment of the information about you in a designated record set for as long as the information is kept by us. To request an amendment, your request must be made in writing and submitted to the Privacy Officer. In addition, you must provide a reason that supports your request.

Right to Request Restrictions

You have the right to request a restriction or limitation on the information we use or disclose about you. To request restrictions, you must make your request in writing to the Privacy Officer. In your request, you must tell us (1) what information you want to limit; (2) whether you want to limit our use, disclosure or both; and (3) to whom you want the limits to apply.

We are required to notify you by first class mail or e-mail, of any breaches of the privacy of the protected health information as soon as possible, but no later than the time period prescribed by privacy regulations.
